We’d like to note that this is not a comprehensive list of Blue Shield of California (BSCA) plan types. Additionally, some BSCA plans do not have mental health benefits built into their plan. If your plan does not have mental health benefits, you will be responsible for the out-of-network rate for all sessions held at Two Chairs.

  • A picture of the front and back of your insurance card
  • Effective date of your current plan
  • Your mailing address
  • Your birthdate
