Any notes or context from previous appointments with a referring provider are not transferred to Two Chairs ahead of your first appointment. Should you feel the need to provide more context to your therapist ahead of the first appointment, please fill out the optional "What else do want your clinician to know?" during your booking process.
You may also reach out to our Care Coordination team after you have scheduled your first appointment and we can help coordinate the transfer of your previous records or information from your referring provider. Your referring provider and your Two Chairs therapist can collaborate as necessary and are considered part of your overall Care Team.
