Support for FMLA & Disability Paperwork

At Two Chairs, our goal is to support you in getting the care you need, including when mental health symptoms may require time away from work or daily responsibilities to facilitate adequate treatment and improvement.

How We Support You

Because Two Chairs offers both Therapy and Psychiatry services, the type of documentation support available depends on the specific provider you are seeing.

Psychiatry Services

If, during your psychiatric treatment, you and your psychiatric provider determine together that you may benefit from medical leave (such as FMLA or short-term disability), your provider can help complete the necessary documentation.

How the Decision Is Made

  • Whether leave is appropriate is a clinical decision made by your provider based on your symptoms, your functioning, your treatment needs and their clinical expertise.
  • Your provider may need multiple sessions to fully assess your condition before deciding whether medical leave is appropriate. This ensures we can make the most accurate and helpful recommendation for your situation.

What to Expect

  • Your provider will talk with you about what symptoms you’re experiencing, how they’re affecting your daily life or work, and how treatment is progressing.
  • If leave is clinically appropriate, your provider can complete the parts of the paperwork that relate to your mental health treatment and are within their scope of practice.
  • Sometimes, a provider may determine that leave is not indicated at this time. In those situations, they’ll discuss alternative treatment options or supports that may better fit your needs.

Important Notes

  • We are able to complete paperwork only for clients who are actively engaged in treatment with a Two Chairs psychiatric provider.
  • We cannot complete disability or FMLA paperwork immediately on request without adequate assessment.

Therapy Services

If, during your therapy treatment, you and your therapy provider determine together that you may benefit from an evaluation for medical leave (such as FMLA or short-term disability), your provider will speak with you about going to your PCP or psychiatric provider for them to complete an assessment and medical determination for you. If you need a referral to one of these professionals your therapist can help you with this. Your therapist may also provide a supportive letter, such as a treatment summary, as deemed clinically appropriate.
